Post-Inflammatory Hyperpigmentation
Post-inflammatory hyperpigmentation (PIH) is skin darkening at sites of previous inflammation or injury. It is especially common in darker skin tones and frequently follows acne or eczema.
Symptoms
Flat, darkened macules at sites of healed inflammation. Color ranges from light brown to blue-gray depending on depth. PIH is not painful but can be slow to resolve.
Causes and risk
Any inflammatory process -- acne, dermatitis, burns, insect bites -- triggers excess melanin deposition. Darker skin phototypes are more susceptible. Picking lesions worsens PIH.
How it is evaluated
Diagnosis is clinical. Wood lamp determines depth. We identify and treat the underlying cause to prevent new PIH.
Treatment
Sunscreen is essential. Topical hydroquinone, retinoids, azelaic acid, and vitamin C accelerate fading. Chemical peels are considered for resistant cases. Our Chicago providers treat the underlying condition concurrently.
